JOB SUMMARY

The Registered Nurse has the responsibility for providing direct and indirect nursing care to assigned patients.  Works under the direct and indirect supervision of the PACE Clinical Services Supervisor or designee but is independent in the application of advanced nursing knowledge and skills.  Manages complex clinical situations for which he/she is responsible.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  1. Assessment: Collects patient data and completes required forms with appropriate responses according to department and industry standards; identifies patient’s overt problems/needs and sets priorities. Identifies problems requiring further referral and/or follow-up; observes and records latest diagnostic results; performs advanced nursing observations using critical thinking skills.
  2. Planning Care: Develops a plan of care based on nursing process and which incorporates the plans of other disciplines and continuing care needs; includes the patient/family in developing or revising the plan of care; and makes referrals to interdisciplinary support services. Initial and periodic assessments (minimally every 6 months) to be completed prior to the scheduled team meetings. Communicates participant changes to team members.
  3. Implementing/Managing Care: Be present whenever medical services are provided; Ensure care being provided conforms to accepted nursing practice standards and organizational/department standards; provide correct treatments/procedures and other care as prescribed and according to patient care standards.  Demonstrate thorough understanding of age-related characteristics, symptoms, and needs of patients served; explain nursing/clinical procedures in appropriate way; provide participant and family education in coordination of 24-hour care delivery; identify emergency situations and independently initiate advanced therapy; act as primary coordinator of patient care with other health care personnel and evaluate patient care measures instituted.  Work with primary care physicians to furnish the preventive, diagnosis, and therapeutic services prescribed for patients accepted for care by the clinic and to implement physician orders as necessary.  Document all nursing treatment accurately and within departmental time requirements into patients’ charts.  Understand and demonstrate respect for patient rights and utilize established mechanisms for management of ethical issues in patient care.
  4. Evaluation: Record patient care delivered as planned and any variation, with appropriate rationale.  Make and record observations, as well as implement nursing measures related to impending or associated problems.
  5. Leadership: Using nursing processes, delegate and supervise the provision of direct patient care by appropriate personnel; integrate cost effective measures into nursing practice; recognize unit problems and take responsibility for documentation; demonstrate active participation in QI processes; comply with organizational expectations to meet staffing demands based on patient care needs; comply with policies addressing safe working conditions; monitor unsafe working conditions and recognizes inappropriate and/or ineffective patient care management, resolve issues/problems; complete written reports; fulfill call for unit as assigned and adjust staffing for volume and acuity as necessary.
  6. Interdisciplinary Team:  Participate as an active and cooperative member of the Interdisciplinary Team.  Attend staff meetings and take part in participant care planning.
  7. Other projects and duties as assigned.

REQUIREMENTS

  • Graduate of an accredited school of nursing.
  • Current RN license in good standing to practice in the required state.
  • Minimum 3 years RN clinical experience. Relevant experience must be within the last 3 years.
  • Experience with paperless documentation preferred.
  • 1 year of experience working with a frail or elderly population preferred. If this is not present, training on working with a frail or elderly population will be provided upon hiring.
  • Must have reliable transportation, a valid driver's license, and the minimum state required liability auto insurance.
  • Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.
  • Pass a comprehensive criminal background check that may include, but is not limited to, federal and state Medicare/Medicaid exclusion lists, criminal history, education verification, license verification, reference check, and drug screen
