Physical Therapist - Upham's PACE

DUTIES AND RESPONSIBILITIES: 

• This position is flexible and part time approximately 15-20 hours per week.
• No evenings, weekends or holidays.
• Provide physical therapy assessments, reassessments, screens and interventions to PACE participants to promote and maximize physical and functional mobility.
• Conduct initial physical therapy evaluations, reassessments, and screens of PACE participants in accordance with PACE guidelines Educate and supervise staff, participants and family members regarding physical therapy rehabilitation principles.
• Conduct home safety and community re-entry assessments Order durable medical equipment and recommend environmental modifications to maximize functional mobility within the home.
• Provide adequate documentation including but not limited to safety evaluations, assessments, screens, treatment plans, progress notes in accordance with PACE guidelines.
• Participate in daily and weekly clinical team meetings.
• Foster participant confidentiality.
• Contribute to quality improvement committees and program development projects.
• Delegate clinical responsibilities appropriately.
• Participate in rehabilitation meetings.
• Delegate and implement appropriate treatment plans for Physical Therapy Assistants and Restorative Aides Actively seek opportunities for professional growth as a physical therapist in the adult and geriatric population.
• Performs duties as requested by Supervisor.
