Your Role:

  • Provide primary care in a concierge setting, working collaboratively with a nurse practitioner. 
  • Work closely with an interdisciplinary team empowered to develop creative solutions and allocate resources that enable vulnerable older adults to age in place at home with dignity and well-being.
  • Practice across the entire continuum of care day-center-based clinics, home visits, skilled nursing facilities, hospital, and end-of-life care. 
  • Enjoy a flexible role that respects your autonomy and makes the most of your comprehensive skill set as a generalist.

What We Look For:

  • Licensed Professionals: MD or DO licensed in Massachusetts.
  • Specialization: Board certification in Internal Medicine or Family Medicine.
  • Interest and experience in Geriatrics: Training in Geriatric Medicine is an added advantage.
  • Licensure Requirements: Active DEA and MCSR licensure.
  • Medical Versatility: Capability to manage a wide range of chronic and acute conditions. 
  • Adaptability: Comfort with interdisciplinary care models and managed care settings. 
  • Cultural Sensitivity: Ease in working within a diverse, urban environment.
